The Advance Health Care Directive Removal Form for the North Carolina Secretary of State allows individuals to withdraw their health care directive documents from the state's official database. It applies to various health care directives, including health care power of attorney, living wills (advance directives for a natural death), advance instructions for mental health treatment, and anatomical gift declarations. The registrant, or a representative in the case of a deceased registrant, must complete this form, have the signature notarized, and submit it to the Secretary of State's office.
