Continue - Drop Form {PL-FL005}

PL-FL005 - CONTINUE/DROP FORM. This form is used to request a continuance or drop of a court hearing. The form requires the case number, fax number, today's date, hearing date and time, hearing department, phone number, and email address. If the parties agree to a continuance or drop, they need to check the appropriate box and attach a written agreement or confirmation letter. If the responding party has not been served, a copy of the filed document must be served with the original hearing's moving papers. The form also allows the requesting party to provide alternative dates for the continuance and specifies that the selected date and time for the continued hearing will be faxed back to the submitting party.
