Forms 08TA012E - COOPERATION AGREEMENT AND REQUEST FOR GOOD CAUSE. You are required, as a condition of eligibility for Temporary Assistance for Needy Families (TANF) and under certain circumstances, SoonerCare (Medicaid) benefits, to cooperate with Oklahoma Human Services (OKDHS) Child Support Services (CSS) to identify and locate the non-custodial parent by providing his or her name, date of birth, Social Security number, address, place of employment, and any other useful information; establish paternity when necessary. This may require genetic tests or an affidavit from the alleged father acknowledging paternity; and obtain child support payments, medical or child care support, or other property and payments as appropriate. Cooperating with CSS may result in CSS locating the non-custodial parent, establishing paternity, and obtaining child support. Once paternity is established, you and your child(ren) may be eligible for child support and other government benefits, such as Social Security or Veterans benefits. If you do not cooperate with CSS, your TANF benefits may be reduced by 25 percent and you may be removed from receiving SoonerCare (Medicaid) benefits. www.FormsWorkflow.com
