Description

Form GF-12-e - PROTECTIVE ORDER PERMITTING LIMITED RELEASE OF FORENSIC EVALUATION. This is used in New York Family Court to authorize the limited release of a forensic evaluation report and its underlying materials and data. The court issues this order to protect the safety, welfare, and best interests of the subject children and the privacy of the parties involved in the forensic evaluation. The order specifies who is authorized to release the forensic evaluation report and to whom it may be released, as well as the specific purposes for which it may be used. It strictly prohibits the re-release, duplication, copying, photographing, transmission, or dissemination of the report and materials without a written court order. The recipient of the report must file an affidavit of non-disclosure with the court within five days of receipt and return the report and materials to the specified entity when no longer needed for the authorized purposes.
