This form is used in real estate transactions by an individual involved in the transaction (seller, purchaser, lessor, lessee, borrower, etc.) to declare whether there is any agreement with a real estate broker regarding compensation for their services. The form also includes an option for the affiant to state that they are not aware of any such agreement. The purpose of the affidavit is to assure the title insurance underwriter and title agency that there are no outstanding broker liens on the property, thus facilitating the issuance of a title insurance policy without exceptions related to commercial real estate broker liens.
