FORM NFP 114.05 - DOMESTIC/FOREIGN CORPORATION ANNUAL REPORT. This form is used for Domestic/Foreign Corporations under the General Not for Profit Corporation Act to provide updated information about the corporation’s activities and financial status to the state of Illinois. It includes details such as the corporation’s name, registered agent, registered office, date of incorporation/qualification, state of incorporation, names and addresses of the corporation’s officers and directors, type of business the corporation is conducting, and whether the corporation is a Condominium Association, a Cooperative Housing Corporation, or a Homeowner’s Association.
