CCDR 0124 - ORDER FOR RECUSAL. This form is used in the Circuit Court of Cook County, Illinois, Domestic Relations Division, to document an order for judicial recusal. Recusal occurs when a judge voluntarily steps down from a case due to potential bias or conflict of interest, in accordance with Canon 2.11 of the Illinois Code of Judicial Conduct and section 735 ILCS 5/2-1001(a)(1). The form orders the transfer of the case to another judge. If a trial judge recuses themselves, the case is reassigned to a Preliminary Calendar Judge.
