REQUEST FOR GLOBAL ATTORNEY SUBSTITUTION (for attorneys of record in more than 20 cases). This form is used for substituting the name of the original attorney and their firm with the name of a new attorney in all pending Eastern and Western District of Arkansas bankruptcy matters where the original attorney is currently the attorney of record. The form includes the original signature of the attorney of record and the new attorney, along with their respective bar ID numbers and the date. It also provides instructions to mail or email both documents to the U.S. Bankruptcy Court. Once the request is submitted, the court issues a Global Order Substituting Attorneys, granting the request and directing the Clerk of the Court to make the substitution as specified in the request.
