DLC-023 - REGISTRATION OF MINOR FOR EMPLOYMENT. This form is used by employers to register minors (individuals aged 16 or 17) for employment in licensed premises overseen by the Department of Liquor Control in Maui County, Hawaii. The form collects information such as the minor's full legal name, date of birth, age, contact information, physical home address, the trade name of the licensed premises, position, and start date. A corporate officer or authorized agent of the employer must sign the form. After an interview with a Liquor Control Officer, the parent or legal guardian of the minor must also acknowledge and consent to the minor's employment at the specified licensed premises. The Liquor Control Officer then completes a section of the form, explaining the relevant rules to the individuals involved, recommending the registration, and indicating any registration fee paid.
