Description
SC-4017 - NOTICE OF CANCELLATION OF MEDIATION AND STATEMENT OF GOOD CAUSE (Local Rule 1501 et. seq., Family Code §3170). This form is used to officially cancel a scheduled family mediation appointment in the Superior Court of California, County of Santa Barbara. The form is used in family law cases involving custody, visitation, or other related matters where mediation is required by the court. The party canceling the mediation must provide a good cause for the cancellation, such as a legitimate reason that prevents attendance at the scheduled session. The form also includes a declaration to confirm that the party has made an effort to notify the other parent (or their attorney, if represented) about the cancellation and to attempt to reschedule the mediation. If the mediation is canceled without good cause, the court may impose sanctions, such as monetary penalties, on the party responsible for the cancellation. The form serves to document compliance with the court's rules for scheduling or canceling mediation sessions. Mandatory Form. www.FormsWorkflow.com
