IN THE CIRCUIT COURT OF THE SEVENTH JUDICIAL CIRCUIT SANGAMON COUNTY, ILLINOIS IN THE MATTER OF: ) ) , ) Case Number: An Alleged Disabled Adult. ) ORDER APPOINTING GUARDIAN AD LITEM This cause having come on to be heard on the Petition for Appointment of Guardian. It appears to the Court that it is in the best interests of the alleged disabled adult to appoint a Guardian ad Litem. IT IS THEREFORE ORDERED BY THE COURT, that is appointed Guardian ad Litem for the alleged disabled adult, to answer the Petition and report to the Court concerning the Respondent222s bests interests consistent with Section 11a-10 of the Illinois Probate Act (755 ILCS 5/11a-10), to elicit the Respondent222s position concerning the adjudication of disability, the proposed Guardian, and any proposed changes in living arrangements, care, and any other areas of inquiry deemed appropriate. The Guardian ad Litem may inspect and copy any medical or mental health records and investigate assets and copy any financial and banking records the Guardian ad Litem deems necessary. The Guardian ad Litem shall personally interview the Respondent prior to the hearing and inform Respondent personally and in writing of the contents of the Petition and of his/her rights under Section 11a-11 of the Probate Act (755 ILCS 5/11a-11).
