ADOPT-205, Declaration Confirming Parentage in Stepparent Adoption, this form is used to tell the court that you are adopting the child of your spouse or domestic partner and also how the child was conceived. Once completed this form would be attached to the Adoption Request (ADOPT-200). This optional form may be attached to the form ADOPT-200 if the adopting parent was married to or in a state-registered domestic partnership with the parent who gave birth to the child at the time the child was born. You may instead attach a declaration in another format containing substantially the same information. The birth parent and the adopting parent must complete separate declarations.
