CK 021, ADVISEMENT FORM, VISITATION: The court advises the parent as follows: During the course of a visitation with your child, you shall not take the child outside of the seven Southern California counties, the State of California, or the United States, without prior court permission. Failure to return the child to his or her placement at the agreed upon time could result in the issuance of a warrant for your arrest, termination of your visitation privileges and even criminal prosecution. RETURN TO PARENT: The court advises the parent as follows: If the child is returned to your care, court jurisdiction continues and the child is still under the jurisdiction of the court. Until this case is closed, you shall not change your residence or that of the child without notifying the social worker. In addition, you shall not take the child outside of the seven Southern California counties, the State of California, or the United Sates, without the permission of the court. Violation of any of these orders could result in the issuance of a warrant for your arrest, removal of the child from your home, and even criminal prosecution.
