FORM 6 - REPRESENTATION STATEMENT. Attach this form or its equivalent to your notice of appeal for filing in the U.S. District Court. Counsel in civil appeals must file a representation statement. Self-represented litigants and counsel in other cases are encouraged to do so but are not required. The purpose of this form is to inform the 9th Circuit Court of Appeals who the parties and counsel are for the appeal. List the name of each party taking the appeal (do not use “et al.”) and any and all attorneys representing that party for the appeal. List the name of each party against whom the appeal is filed, as well as every attorney representing that party.
