The form requests the following information: • Case Number: The unique identification number assigned to the case. • Case Name: The name of the case to which the lodged documents relate. • Related Filing: Information about the filing that is related to the lodged documents. • Party Filing Lodgment (name): The name of the party who is filing the lodged documents. The form also includes a section to specify the means of return of the lodged documents, such as Attorney Service Pick-Up, Self-Addressed Stamped Envelope, or Destroy.
