SUPCV 1041- APPLICATION AND ORDER FOR PUBLICATION OF SUMMONS OR CITATION. This form is used to request and obtain a court order for service by publication when a party, such as a defendant or respondent, cannot be located for personal service in a legal case. It is typically applied for in civil, probate, or citation cases where reasonable attempts to serve the party in another manner have been unsuccessful. The applicant, who is either the plaintiff, petitioner, or their attorney, must demonstrate that diligent efforts were made to locate and serve the individual.
