ADULT CONSERVATORSHIP AND ADULT GUARDIANSHIP (PACKET). This packet of forms are used in the Richland County Probate Court for Adult Conservatorship and Guardianship to start the legal process of appointing a conservator to manage finances and/or a guardian to manage personal care for an incapacitated adult. The petition and summons are filed with a fee, and other required documents include a notice of the right to legal counsel, a medical examiner's report on the person's capacity, background checks, credit reports for the proposed conservator, and identification documents for both the conservator and the alleged incapacitated individual. A current picture of the incapacitated individual is also required. All interested parties must be served with these documents, and proof of service must be provided through acceptance of service, certified mail, or personal service.
