PROBATE COURT OF MONTGOMERY COUNTY, OHIO ALICE O. McCOLLUM, JUDGE GUARDIANSHIP OF: CASE NO.: JUDGMENT ENTRY 30 DAY EXTENSION OF EMERGENCY GUARDIANSHIP The undersigned Judge has reviewed the Magistrate's Decision filed in this case and finds that there are no errors of law or defects evident on the face of the decision. Therefore, the undersigned Judge adopts the Magistrate's Decision as the decision of this Court. This case came before the Court for hearing on the , 20 day of , on the issue of whether the emergency guardianship for the above-named ward should be extended for a period of thirty (30) days. A copy of the order appointing the emergency guardian and setting this issue for hearing was served upon the ward and those persons entitled to notice of the application and hearing on the day of , 20 . On the basis of the evidence presented at the hearing, the Court finds that the circumstances necessitating the emergency guardianship continue to exist and that a thirty-day (30) extension of the emergency guardianship is necessary to prevent significant injury to the person and/or estate of the ward. Therefore, for good cause shown, the Court extends the emergency guardianship until , 20 , subject to further order of the Court. The Court ORDERS that Letters of Guardianship be issued to the emergency guardian. The Court further ORDERS that a copy of this Entry and the Magistrate's Decision be served upon the ward and any other persons entitled to notice of the application and hearing. DATE ALICE O.
