IN THE CIRCUIT COURT OF THE TWELFTH JUDICIAL CIRCUIT WILL COUNTY, ILLINOIS IN RE: THE MATTER OF: Plaintiff vs Defendant ADMISSION OF PATERNITY THIS CAUSE corning to be heard on establishment of paternity, the Court having jurisdiction, the father having appeared and after being advised by the Court of the nature of the Petition filed in this matter, and of his rights under the Illinois Parentage Act, specifically that the father has the right to: Obtain Counsel; Notice of all proceedings instituted to establish paternity; Request deoxyribonucleic acid (DNA) testing for the determination of inherited characteristics; Request Experts to conduct and interpret said DNA test results and prepare a written document regarding the same, to which they will testify to the nature of such results if deem necessary by this Court; 5. Request the Court to order the expenses of said DNA test be paid by either the person requesting the DNA testing or if the person is found to be indigent, the County of Will; 6. Request the Court to conduct a bench trial on the issue of paternity, custody, and/or visitation; 7. Request a free transcript of these proceedings, for an appeal, should Respondent be found indigent. IT IS HEREBY ORDERED THAT: The undersigned hereby acknowledges that he is the father of the minor child identified below and is aware of the consequences of his admission of paternity, namely the entry of an order compelling him to pay future support, retroactive support as supported by the evidence, provide medical/hospitalization insurance, and to reimburse the Petitioner for any amounts paid on behalf of the minor child from the birth of the child, including pregnancy and the delivery expenses.
