NC-311, BIRTH CERTIFICATE FOR CHILD OF PETITIONER—ATTACHMENT, Attachment to Petition for Recognition of Change of Gender and Sex Identifier, Name Change, and Issuance of New Certificates (form NC-300). Petitioner must complete and attach this form to form NC-300 if asking the court to order issuance of a new birth certificate of a minor or adult child that reflects petitioner's change in gender, or change in both gender and name.
