147 - SUMMONS 5 DAY CORPORATE SERVICE (SUMMARY PROCEDURE PURSUANT TO F.S. 51.011). This form is used in either the Circuit Court of the Eleventh Judicial Circuit or the County Court in Miami-Dade County, Florida. It is intended for cases where the Summary Procedure is being followed pursuant to Florida Statute 51.011.
