GN-4065 - NOTICE OF FIREARM RESTRICTION. This form is used in Wisconsin's Circuit Court to notify an individual about a firearm restriction placed upon them. The form informs the individual that they are prohibited from possessing any type of firearm and explains the potential legal consequences of violating this prohibition. It specifies that violating the prohibition can result in a Class G felony charge, which carries a fine of up to $25,000, imprisonment for up to 10 years, or both. The restriction will remain in effect until lifted by the court. The individual is also informed that they have the option to petition the court to cancel the firearm restriction order.
