PCM 217a - ORDER REGARDING REQUEST TO MODIFY ORDER FOR ASSISTED OUTPATIENT TREATMENT OR COMBINED HOSPITALIZATION AND ASSISTED OUTPATIENT TREATMENT. This form is used in the State of Michigan Probate Court. The order pertains to an individual who was previously directed to undergo a program of assisted outpatient treatment or combined hospitalization and assisted outpatient treatment.
