JD-ES-145 - LAW FIRM JURIS NUMBER APPLICATION OR CHANGES. Issuance of a Firm Juris Number does not constitute registration of the law firm with or recognition of such law firm by the Judicial Branch. The terms “law firm” and “firm” as used in this form mean a legal entity whose primary purpose is to engage in the practice of law in Connecticut or elsewhere as that term is defined by Practice Book Section 2-44A. Firm juris numbers are issued per law firm, not per individual attorney, position, or office within that law firm. If a law firm has multiple locations in different cities, states, or countries each location may be issued a firm juris number. The attorney applying for a firm juris number must certify to the criteria in Section One of this form in order for a firm juris number to be issued.
