This form is used to provide individuals with hearing impairments or limited English proficiency the opportunity to waive their right to receive auxiliary aids, interpretive services, or translator services during court proceedings. Auxiliary aids and services, interpreters, and translators are typically provided to ensure effective communication and participation of individuals who have difficulty understanding the proceedings due to hearing impairments or language barriers. However, this form allows these individuals to voluntarily waive their right to such services.
