CCP 1010 - WRITTEN NOTICE TO REPRESENTATIVE ELECTING OR REQUESTING SURVIVING SPOUSE’S AWARD OR CHILD’S AWARD. This form is used in the Circuit Court of Cook County, Illinois, Probate Division for estates of deceased individuals. It is sent to the representative of the estate, typically through their attorney. The form allows the surviving spouse or other eligible individuals to elect or request a sum of money as a surviving spouse's award or child's award from the estate of the deceased.
