BWC-1360 - AUTHORIZATION TO RECEIVE WORKERS' COMPENSATION CHECK (C-230). This form is used for injured workers or claimants to authorize and direct the Bureau of Workers' Compensation (BWC) to mail compensation checks directly to their attorney. The form is used for specific compensation claims and must be completed in its entirety, including the correct claim number, signature, and date. It provides authorization for the attorney to receive the compensation check on behalf of the injured worker for the types of compensation listed in the form. The completed form can be submitted to BWC either by fax or mail. The authorization is only valid for a specified period and does not continue in effect after BWC has paid the awarded compensation unless there is a subsequent hearing, appeal, or reconsideration after payment.
