Form SURR-4 - AFFIRMATION OF AUTHORIZED AGENCY EMPLOYEE WITNESS TO EXECUTION OF EXTRA-JUDICIAL SURRENDER OF CHILD IN FOSTER CARE. This form is used in the Family Court of the State of New York for the application for approval of a surrender instrument concerning a child. The affirmation is made by an employee of an authorized agency who witnessed the execution and acknowledgment of the surrender of a child in foster care. The employee affirms that they were trained to receive surrenders in accordance with regulations and provides details of the surrender, including the date, location, and individual executing the surrender.
