DLC 4115 - F PERMIT APPLICATION. Allows qualifying organizations to sell beer ONLY until 1am for up to 5 days. Application MUST be filed with us AT LEAST thirty (30) days PRIOR to the event date or your application may NOT be approved in time for your event. For your application to be deemed complete, you must fully and legibly complete this application, including: • Answering all required questions (“*” indicates a required field); • Paying all the required fees listed below; and • Submitting any additional required forms listed below.
