18 - PERIODIC REPORT. This form is used to provide updates on workers' compensation payments to the South Carolina Workers’ Compensation Commission. It includes information about the claimant, employer, insurance carrier, and preparer. Additionally, it requires details such as the date of injury, total weeks of compensation paid, types of compensation paid, periods of payment, date of the first payment, and total amount paid for compensation and medical expenses. The form must be filed every six months after the alleged injury date until the Commission’s file is closed, regardless of whether compensation is ongoing.
