GC-20 - PROPOSED GUARDIAN(S) INFORMATION FORM. This form is used in the Superior Court of California, Probate Investigations Unit of Contra Costa County. The purpose is to obtain consent from individuals living in a household, aged 18 years or older, to release information to the Probate Investigations Unit regarding any records that Children and Family Services may have pertaining to them. The form includes sections for individuals to provide their consent, along with their signatures and dates. Please complete this entire form and return it to the Probate Investigations Unit, within five days. Use additional paper, when necessary, to answer questions. Per Local Rule 7.400 Probate Investigations Unit will initiate a guardianship investigation except when the court specifically directs otherwise, only after the petitioner(s) has submitted a complete “Proposed Guardianship Information” www.FormsWorkflow.com.
