FORM 2 - APPLICATION FOR INFORMAL PROBATE OF WILL AND APPOINTMENT OF A PERSONAL REPRESENTATIVE. (N.D.C.C. 30.1-14-01). This form is used when a person dies and someone is applying to be appointed as the personal representative of the deceased’s estate. The applicant provides their name and interest, details about the decedent (the person who died), including their date of death, age, domicile, and information about the surviving spouse, children, heirs, and devisees. The applicant also states that no personal representative has been appointed whose letters have not been terminated, and that they have not received any demand for notice of any probate or appointment proceeding concerning the decedent. The applicant further states that after exercising reasonable diligence, they are unaware of any instrument revoking the will and they believe the instrument is the decedent’s last will. The applicant requests that the will be admitted to informal probate, and that they be appointed personal representative of the decedent’s estate.
