NON-MILITARY AFFIDAVIT. This form is used in the County Court of the Fifth Judicial Circuit of the State of Florida, Hernando County by a plaintiff in a case against a defendant to affirm that the defendant is not in the military service or any governmental agency subject to the provisions of the Soldiers’ and Sailors’ Civil Relief Act. The affiant, usually the plaintiff or their representative, declares under oath that the defendant is not in the military. The form includes spaces for the affiant's signature, name, address, and telephone number, as well as a space for the signature of a deputy clerk or notary public.
