SIE-0038 - PERSONALIZED LIST OF RIGHTS. This form is used in conservatorship cases in the Superior Court of the State of California, County of Sierra. It informs the individual under conservatorship of their rights despite the establishment of the conservatorship. These rights include the ability to understand, make, and communicate their own choices, be informed about decisions made by their conservator, remain as independent as possible, make or change their will, enter into reasonable business transactions, have a lawyer, request a judge review concerns or complaints, ask for changes in conservators, and request termination of the conservatorship. The form lists various personal and estate rights, and indicates whether the individual retains each right (marked "Yes"), has had it changed ("Changed"), or does not have the right ("No"). Rights include receiving personal mail, visits from family and friends, marrying or entering into a domestic partnership, making health care decisions, deciding on medication for neurocognitive disorder, voting, selecting residence, accessing confidential records, making educational decisions, receiving and controlling wages, entering business transactions, managing property, collecting and managing claims, and paying or defending claims.
