CONSENT TO PROCEED BEFORE A SINGLE ARBITRATOR. This form is used in the United States District Court for the Eastern District of Pennsylvania for parties in a civil action to voluntarily agree to have their arbitration trial conducted before a single arbitrator, instead of a panel of three arbitrators, in accordance with Local Civil Rule 53.2, subsection 2. The form requires the signatures of the parties or their attorneys, indicating their consent. The completed form is then electronically filed or emailed to the court, or mailed if electronic means are not available. This agreement to proceed before a single arbitrator is optional.
