STATEMENT OF CONSENT TO PROCEED BEFORE A UNITED STATES MAGISTRATE JUDGE (For use in Direct Assignment of Civil Cases to Magistrate Judges Only). This consent form shall be used only for cases in which a magistrate judge is initially assigned pursuant to standing order 2022-19. This form is for the United States District Court for the Middle District of Pennsylvania, used to obtain consent from parties in a civil case to proceed before a United States Magistrate Judge. It offers two options: parties can either consent to allow the Magistrate Judge to conduct all proceedings, including trial and final judgment, or choose not to consent, which will result in the case being reassigned to a District Judge. Parties must return the form within 30 days of receipt to indicate their choice.
