FORM 5.0 - APPLICATION TO RELIEVE ESTATE FROM ADMINISTRATION. This form is used to request that an estate be relieved from full administration under Ohio Revised Code 2113.03 because its total assets fall below certain statutory thresholds. This process, called Relief from Administration, is a simplified probate procedure that allows for a quicker and less costly transfer of assets without the need for a formal probate case. The applicant must show that the total value of the decedent's assets falls within the allowable limits based on the date of death. The form includes information about the decedent, their assets, and the applicant's request to be appointed as commissioner to distribute the estate. If applicable, it also includes a waiver of notice signed by heirs, legatees, or other interested parties.
