P-415 - ADOPTION CONSENT (AGREEMENT) Parent, Spouse, or Guardian. This form is used in the Superior Court for the State of Alaska for a parent, spouse, or legal guardian to provide consent to the adoption of a child. The person signing the form must acknowledge their relationship to the child, understand the legal implications of the adoption, and voluntarily consent to it. Depending on their relationship with the child, they may retain certain rights, such as visitation privileges or inheritance rights, or permanently give up all rights and responsibilities for the child.
