Description
PRELIMINARY CONFERENCE STIPULATION AND ORDER (Sections 202.2 and 202.12 of the Uniform Rules). This form is used in the Supreme Court of Suffolk County as part of the Differentiated Case Management (DCM) process. It sets forth a schedule and guidelines for the disclosure process in a civil case, designating the case as "standard" and subject to a 12-month disclosure period. The form outlines specific deadlines and requirements for various aspects of pre-trial procedures, including the exchange of insurance coverage information, demands for and responses to a bill of particulars, medical reports and authorizations, physical examinations, depositions, and other discovery-related activities. It also includes provisions for motions to amend pleadings or add parties, scheduling a compliance conference, and setting an end date for all disclosure other than expert disclosure. The form is signed by the attorneys for all parties involved and is so ordered by the presiding judge. Failure to comply with the stipulated directives may result in sanctions or other legal consequences. www.FormsWorkflow.com
