AOC-710 - VERIFIED PETITION FOR INVOLUNTARY HOSPITALIZATION (Mental Illness) OR INVOLUNTARY ADMISSION (Intellectual Disability). This form is used in Kentucky to request the involuntary hospitalization of an individual due to mental illness or intellectual disability. A petitioner, who could be a concerned individual or a qualified mental health or intellectual disabilities professional, submits this form to the court. The petitioner must state their relationship to the respondent, provide reasons for believing the respondent is mentally ill or intellectually disabled, and explain why the respondent poses a danger to themselves or others. If the petition is related to intellectual disability, documentation supporting the diagnosis must be attached. The petitioner requests the court to order the respondent's detention for examination, evaluation, and possible hospitalization or admission for a specified period.
