FORM 11 - IRREVOCABLE CONSENT TO SERVICE OF PROCESS BY NON-RESIDENT APPLICANT FOR REGISTRATION OF SECURITIES UNDER CHAPTER 1707. This form is used by a non-resident applicant seeking to register or exempt the sale of securities in Ohio. The form allows the issuer of the securities to provide an irrevocable consent to service of process in the event of a lawsuit arising from the sale of those securities, or in cases of fraud related to the sale of securities. By signing this form, the issuer agrees that legal actions may be initiated in any Ohio county, and that service of process can be made on the Secretary of State of Ohio, who will act as the issuer's agent for receiving legal documents.
