JV-001 - DECLARATION FOR ACCESS TO JUVENILE CASE FILE IN POSSESSION OF JUVENILE COURT (Welfare & Institutions Code § 827, California Rules of Court, rule 5.552). This form is used to request access to a juvenile case file in possession of the Juvenile Court of the Superior Court of California, County of Stanislaus. The form requires the requester to provide their name and other identifying information, such as their relationship to the case or their capacity as described in Welfare and Institutions Code section 827. The form includes a list of individuals and entities who are entitled to inspect and receive copies of juvenile case files, as well as those who are entitled to inspect the files without receiving copies. The requester must indicate which category they fall under and specify the documents they need copies of, if applicable. The form also includes declarations that the requester will not share the records or information from the case file, except as provided by law, and that they will not attach any portion of the case file to any other document without prior approval.
