CC 236 - ORDER COMMITTING JUVENILE TO MICHIGAN DEPARTMENT OF HEALTH AND HUMAN SERVICES.. This form is used in Michigan's Judicial Circuit Court for cases involving juveniles who have been found guilty of a crime. It is an order committing the juvenile to the Michigan Department of Health and Human Services (MDHHS) for probation and treatment. The form contains various findings and orders related to the juvenile's conviction, probation, and placement. The form includes the juvenile's personal information and details of their conviction. It specifies if the conviction involves offenses that require reporting to the Secretary of State or licensing sanctions. The court may also order HIV testing and sex offender registration if necessary. The juvenile's fingerprinting is required, except in cases where their DNA sample is already on file with the Michigan State Police. The court places the juvenile on probation and commits them to MDHHS for supervision. The Director of MDHHS is appointed as a special guardian to receive any benefits due to the juvenile from the U.S. government. Temporary custody arrangements are determined while waiting for transfer to MDHHS. The form states the initial placement of the juvenile, whether in a treatment program, home, or facility. The Michigan Department of Health and Human Services is obligated to provide semiannual progress reports on the juvenile's status. The court is notified in advance to schedule a commitment review hearing before the juvenile's 19th birthday. The Michigan Department of Health and Human Services must forward a commitment review report for the hearing. The form specifies reimbursement for the cost of care and services and includes an order for the juvenile to pay certain costs like victim restitution, DNA assessment, court costs, attorney fees, and fines.
