POST-DISTRIBUTION ACCOUNTING FORM. This form is used in the United States District Court, Northern District of California in class action settlements to report on the distribution of settlement funds. Within 21 days after the settlement checks become stale (or, if no checks are issued, all funds have been paid to class members, cy pres beneficiaries, and others pursuant to the settlement agreement), the parties should file a Post-Distribution Accounting (and post it on the settlement website). It tracks important metrics such as the total settlement fund, the number of class members who received notice, claims submitted, opt-outs, objections, and the amount each claimant received. It also records the methods used to notify and pay class members, the amount of funds unclaimed or undistributed, payments to cy pres recipients (organizations designated to receive unclaimed funds), and administrative costs like attorney fees.
