ORDER TO EXAMINE SAFE DEPOSIT BOX. This form is used in the Surrogate's Court of Queens County, New York, to authorize the examination of a deceased person's safe deposit box. The order is issued upon a petition requesting access to the box to determine if it contains a last will and testament, insurance policies, or a burial plot deed. If a will is found, it must be delivered to the Clerk of the Court. If an insurance policy payable to a named beneficiary is located, it must be given to the beneficiary. If a burial plot deed is found, it is to be delivered to the petitioner or their agent.
