JD-CL-12 - APPEARANCE. This form is used in the Connecticut Superior Court system to formally notify the court, attorneys, and self-represented parties that an individual or attorney is representing a party in a case. It is applicable in civil, family, housing, small claims, criminal, and motor vehicle matters. This form is also used to update a self-represented party’s address if it has changed. Attorneys and self-represented litigants must file this form to establish their representation in a case or to indicate a change in representation. The form allows a party to specify whether they are appearing for the plaintiff, defendant, or another role in the case. It provides options for attorneys who are replacing another attorney's appearance or filing in addition to an existing appearance. For family matters, the form distinguishes between general family court issues and Title IV-D child support matters. In criminal and motor vehicle cases, it can also be used by public defenders, assigned counsel, or special public defenders for specific hearings such as bail hearings or alternative arraignment proceedings. The form must be filed with the court and served on all parties, and attorneys who are not exempt from e-filing must submit it electronically. Self-represented individuals must also notify the court and other parties if their mailing address changes by submitting an updated appearance form.
