IN THE LIMA MUNICIPAL COURT OF LIMA, OHIO ______________________________________ PLAINTIFF -vs______________________________________ CASE NO. __________________ RELEASE WHEREAS, the undersigned is a party in this action, and WHEREAS, said party has requested assistance from the Clerk or Deputy Clerks of this Court in the preparation and or filing of proceedings in this case, and WHEREAS, said party understands that neither the Clerk of this Court or any of the Deputy Clerks of this Court are attorneys at law or are licensed to practice law in the State of Ohio or any other state, NOW, THEREFORE, said party hereby acknowledges that the Clerk of this Court and the Deputy Clerks thereof are not attorneys at law and are not licensed to practice law in the State of Ohio or any other state and that any advice which said party may receive from said Clerk or Deputy Clerks is strictly the advices of a layman. Said party further hereby releases the Clerk of this Court and the Deputy Clerks of this Court from any liability whatsoever for any advice or assistance given in regard to this case or the preparation and/or filing of any proceedings herein.
