That on a Judgment of Dissolution of Marriage was entered, and the Judgment provided the following: that the Plaintiff Defendant Parties was/were granted sole custody/joint custody of the minor child(ren) of the parties; that the Plaintiff Defendant was ordered to pay child support in the amount of $ per week bi-weekly month twice a month other for the support of minor child(ren); 2. That since the entry of the judgment, there has been the following subst antial change in circumstances: that since the last support order was entered, there has been a substant ial increase decrease in the income of the Plaintiff Defendant, in that that due to the increased ages of the children of the parties, expenses for their support, including but not limited to clothing, food, activities, school expenses, etc., have subst antially increased; WHEREFORE , the Plaintiff Defendant prays that this Honorable Court as follows: A. That the Judgment for Dissolution of marriage be modified to provide tha t the child support bereduced increased to the percentage of net income as provided by statute; B. For such other and further relief as this Court deems just and equitable .
