SUM-100, SUMMONS, this form is used to tell a party being sued (the defendant) that a court case has started and what can happen if a response is not filed in 30 days. This form is used to notify a defendant that there is legal action being taken against them. It will notify the defendant where they need to appear and when in order to defend themselves or appeal the decision. The form will list the name and contact information for the defendant and the plaintiff and will state that the defendant is being sued, without listing any details. It will state the court where the proceedings will be carried out, the date and the case number. The contact information for the plaintiff or the plaintiff’s attorney will also be included so that the proceedings can be scheduled with both parties.
