BILL OF COSTS. This form is used in the United States Bankruptcy Court for the Western District of Wisconsin by the prevailing party in a bankruptcy case or adversary proceeding to request reimbursement for litigation-related expenses. After judgment has been entered, the party seeking costs submits this form to the bankruptcy clerk, listing specific categories of allowable expenses—such as filing fees, service of process, transcript preparation, printing, witness fees, copying charges, and deposition costs. The requesting attorney must declare under penalty of perjury that all listed costs were necessarily incurred and services were actually performed. A copy of the completed form must also be mailed to the judgment debtor. Once reviewed, the clerk may approve and tax the costs, which are then added to the judgment amount.
